Freckles are small, multiple brown spots that appear on the skin and are common in people with fair skin. They are caused by an increase in the amount of the pigment-producing cells called melanocytes as well as the amount of the dark pigment called melanin.

Ephelides and lentigines freckles can occur in the same person, and the risk factors for both are typically the same. Ephelides freckles appear during the summer due to sun exposure, and they disappear during the winter. Lentigines freckles are darker in color and are more permanent in nature.

Ephelides, an inherited trait that also occasionally affects people with darker skin types, are very common in fair-skinned people, especially in children with red hair, where it is believed that the MCIR gene is the main gene involved.

Lentigines are common after the age of 40, but they may also occur in younger individuals. Lentigines are common in individuals with fair skin, but they also frequently arise in sun-exposed sites in individuals who frequently tan or who have naturally dark skin.

Lentigines are brown flat lesions with a distinct edge; the most common type, solar lentigines, appear in middle age and are brought on by sun damage; they are more noticeable and larger than freckles, and they are most frequently found on the hands and face.

Causes

An ephelis is brown because of the pigment melanin, which is produced by melanocytes and diffused into keratinocytes. Melanin production by melanocytes increases when the skin is exposed to UV radiation in sunlight, and it increases locally in keratinocytes without increasing melanocyte number.

Symptoms

Ephelides are typically less than 3mm in diameter and appear on a person’s mid-face and occasionally more widely as they age. Ephelides are noticeable in the summer but significantly fade or disappear in the winter.

The lesion’s border is clearly defined, and an irregular border may give it a scalloped shape. They range in size from a few millimeters to several centimeters in diameter.
