Because thyroid hormone affects growth, development, and numerous cellular processes, inadequate thyroid hormone has far-reaching effects on the body. Hypothyroidism is a condition characterized by abnormally low thyroid hormone production.

Thyroid hormone regulation- the chain of command

The pituitary, which is a brain-based gland, is in turn controlled by the thyroid (via the “feedback” effect of thyroid hormone on the pituitary gland), as well as by the hypothalamus and another brain-based gland called the pituitary, which in turn is controlled by another brain-based gland called the hypothalamus.

Thyrotropin-releasing hormone (TRH) is a hormone released by the hypothalamus that signals the pituitary to release thyroid stimulating hormone (TSH), which then signals the thyroid to release thyroid hormones. If there is an imbalance at any of these levels, thyroid hormone production may be impaired, leading to hypothyroidism.

The pituitary gland regulates the rate at which thyroid hormone is produced; if there is not enough thyroid hormone present to support normal functioning, the pituitary increases the release of TSH in an effort to stimulate more thyroid hormone production; conversely, if there is too much thyroid hormone present, the pituitary decreases thyroid hormone production by lowering TSH levels.

Autoimmune Thyroiditis (Hashimoto’s Thyroiditis)

Hashimoto’s thyroiditis is an autoimmune disease in which the body attacks its own thyroid gland, resulting in a goiter and progressive thyroid destruction. It is the most prevalent inherited cause of hypothyroidism in adults.

Subacute Thyroiditis (inflammation of the thyroid gland after a viral disease)

The three types of subacute thyroiditis are subacute granulomatous, also known as painful thyroiditis; subacute painless thyroiditis, which is silent and also known as lymphocytic thyroiditis; and postpartum thyroiditis. In this condition, there is typically a phase of hyperthyroidism (a condition in which the thyroid produces an excess of thyroid hormones), followed by a hypothyroid phase, before thyroid function returns to normal.

Previous Thyroid Therapy

To treat thyroid nodules or other conditions, the thyroid gland may be surgically removed. Radioactive iodine therapy for hyperthyroidism also results in thyroid tissue destruction and may cause hypothyroidism.

Drug-induced Hypothyroidism

Lithium (Eskalith, Lithobid) and amiodarone (Cordarone) are two examples of prescription drugs that may affect thyroid function.

Pituitary and Hypothalamic disease

Hypothyroidism caused by pituitary disease is known as “secondary hypothyroidism,” whereas hypothyroidism caused by a hypothalamic disease is known as “tertiary hypothyroidism,” because both the hypothalamus and pituitary gland are involved in the signaling pathways that regulate the function of the thyroid gland.

Iodine deficiency

Iodine deficiency does not exist in the United States; however, in mountainous regions of developing, less developed countries, severe iodine deficiency may result in low thyroid hormone levels.

Symptoms

TSH, or thyroid stimulating hormone, is primarily used to detect hyperthyroidism because it is secreted by the pituitary gland and can be directly measured when hypothyroidism is present.
